  • Must be a Board Certified Chaplain with the Association of Professional Chaplains, National Association of Catholic Chaplains, or Neshama: Association of Jewish Chaplains, or BCC eligible and must attain certification within 2 years of employment.
  • Must hold a Master of Divinity degree or their equivalent from an accredited educational institution.
  • Must be endorsed to serve as a chaplain from a recognized spiritual/faith group.
  • Must have 4 units of clinical pastoral education.
  • Must have experience in providing spiritual care to individuals with a wide variety of physical and cognitive abilities, particularly older adults.
  • Must have the ability to plan, organize and operationalize spiritual and emotional wellness programs for people from diverse cultural, educational and spiritual backgrounds.
  • Must be able to protect confidential information.

JOB OBJECTIVE

The MDS/Restorative Nurse is responsible for the system to achieve accurate and timely submissions of MDS data and implementing and sustaining Restorative Nursing plans for residents as indicated by their conditions.  Through demonstrated leadership designs these systems to assure that all State and Federal regulations for skilled care are met. Follows all Saint John’s policies, procedures and established standards of care. Role models person first behaviors and holds others accountable to Saint John’s person-centered philosophy of care and service to provide our residents with the power of choice.

Qualifications:

  1. Must be a graduate from an accredited School of Nursing and currently licensed as a Registered Nurse in the State of Wisconsin. Bachelor's of Science in Nursing preferred.
  2. Must have 3 years of experience in Geriatric/rehabilitation nursing required including. One year of supervisory or leadership experience preferred.
  3. CPR certification optional; required if working additional hours in a clinical area.
  4. Must be able to read, speak and write English.

Wellness Specialist

Part-time /1st Shift 

  • Graduate of an accredited physical therapy program with a bachelor’s, master’s, or doctorate physical therapy degree
  • Successful completion of APTA Certification Examination for Physical Therapist. Current Wisconsin licensure as a physical therapist in good standing.
  • Must have continuing education, certification, or a Bachelor’s degree in Exercise Science or related field.  Experience in conducting various specialty group exercise classes and one-on-one personal training, including but not limited to, strength, low impact aerobics, yoga, Pilates, aqua fitness, etc.
  • Experience with the older adult population desired.  Certifications with ASCM, ACE, AFAA, or SFA desired but not necessary.
  • Must have CPR/First Aid certifications or become certified in first 6 months of employment.
